What are the natural (fertility awareness) methods of contraception?
Answer
Natural methods avoid intercourse during the fertile window, identified by:
- Calendar (rhythm/safe period) method
- Basal body temperature method (a small temperature rise after ovulation, from progesterone)
- Cervical mucus (Billings) method
- Symptothermal method (combination)
Also withdrawal (coitus interruptus) and the lactational amenorrhoea method. They are free but less reliable.
Clinical relevance. Knowing the natural methods (fertility awareness, withdrawal, LAM) supports counselling for couples preferring non-hormonal/non-device options.

Natural methods identify the fertile window to time intercourse, avoiding hormones/devices.
Fertility awareness methods
- Calendar (rhythm/safe period) method - estimating the fertile days from cycle length.
- Basal body temperature method - detecting the slight rise in body temperature after ovulation (from progesterone secreted by the corpus luteum).
- Cervical mucus (Billings) method - recognising fertile-type (clear, stretchy) mucus.
- Symptothermal method - combining several natural methods for greater accuracy.
Other natural methods
- Coitus interruptus (withdrawal).
- Abstinence.
- Lactational amenorrhoea method (LAM).
Pros and cons
- Free, no side effects, acceptable to some (e.g. for cultural/religious reasons).
- Less reliable (higher failure rates), require commitment, and do not protect against STIs.
